  Code Pulse Platform — Audit Results

  TLDR: The skeleton is real and the loop's build/test claims are honest —
  everything compiles, 18 platform tests pass, lint is warnings-only. But
  "validated by 4 AI reviewers" oversold it badly. The audit (39 agents, every
  serious finding adversarially verified against the code) confirmed 2 CRITICAL 
  and ~15 HIGH issues in three clusters: the security model is hollow, the
  ingestion pipeline — the core of the product — silently loses and 
  double-counts data, and several flagship features are wired but dead 
  end-to-end. None of the riskiest code has a single test, and no CI runs on
  push at all.

  ---
  1. What's genuinely done and works

  - Daemon core is solid plumbing: coherent HTTP routing, bearer auth on all
  non-exempt endpoints, restrictive CORS/WS origin allowlists, ordered graceful
  shutdown, token file at 0600 (http/server.ts, ws/broadcaster.ts, config.ts).
  - Protocol validation is real: validateEnvelope genuinely rejects malformed
  input across 6+ rejection paths; all SQL is parameterized; migrations are
  idempotent and version-gated.
  - Restore safety design is right: dry-run by default, single-use 60s recovery
  token, path-guard blocks ../ and symlink escapes relative to a given root (the
  root itself is the problem — see below).
  - Desktop app has no token persistence, no {@html} injection surface, honest 
  loading/error/empty states, and a genuinely two-phase restore flow.
  - CLI doctor/status/registry and the MCP server were verified live end-to-end
  — real session/token data, correct exit codes, SDK + fallback both work.
  - Extension diff: session tags work end-to-end, GoalProgress math is correct,
  focus-session crash-safe persistence works, daemon fallback degrades cleanly,
  webview got real CSP + escaping.
  - Claims check (re-ran everything): platform build ✅, 18/18 tests ✅, root
  typecheck ✅, lint 0 errors / 115 warnings ✅. One caveat: the running daemon
  (started yesterday) predates today's rebuild — restart it to run current code.

  2. What's done wrong

  Security (the localhost trust model doesn't hold)

  1. CRITICAL — DNS rebinding steals the bearer token from any web page
  (platform/apps/daemon/src/http/server.ts:204,484-497). The daemon never
  validates the Host header, /v1/bootstrap is auth-exempt, and missing Origin = 
  trusted — which is exactly what a same-origin fetch after DNS rebinding sends.
  A malicious page gets the token, then full API access including disk-writing
  restore. Also exploitable trivially by any local process via curl (defeats the
  0600 token file on multi-user hosts).
  2. CRITICAL — manifest signature verification fails open
  (packages/registry/src/signature.ts:36). Any unsigned manifest is accepted;
  trust: "official" is read from the attacker's own JSON and auto-enables the
  scanner; bundleHash is self-referential so it proves nothing. One dropped JSON
  file = the daemon tails arbitrary files. And the inverse bug: signed
  manifests are rejected (signature.ts:16) while the protocol validator requires
  signatures — the two validators are mutually contradictory, so the real
  registry can never ship.
  3. HIGH — authenticated arbitrary file write (server.ts:385 →
  SnapshotManager.ts:199). projectRoot is caller-controlled, so the path guard
  guards nothing: projectRoot=~, filePath=.ssh/authorized_keys,
  contentBefore=<attacker key>, then confirm restore. Comb with #1 and it's
  remote.

  Ingestion pipeline (the core silently corrupts data)

  4. HIGH — glob baseDir is computed one directory too high
  (scanner/glob.ts:47). path.dirname on a trailing-slash string drops a real
  segment, verified against built code. Result:
  ~/.codex/archived_sessions/*.jsonl matches the wrong files (archive ingestion
  is dead), and ~/.claude/projects/**/*.jsonl escapes to scan all of ~/.claude.
  5. HIGH — 48-file cap applied before sorting (glob.ts:6,130). Survivors are
  whatever readdir returned first, not newest. Heavy Claude Code users silently
  never tail their newest sessions.
  6. HIGH — one bad file permanently stalls the whole poll loop
  (log-tailer.ts:145). fs.open is outside try/catch; an EACCES/EMFILE file (now
  likely, thanks to #4's over-broad glob) throws and unwinds pollManifests —
  every file ordered after it is never tailed again.
  7. HIGH — out-of-range timestamp crashes the parser and wedges the tailer
  (parsers/utils.ts:127, also validate.ts:674). new Date(1e25).toISOString()
  throws; no per-line try/catch, so the poisoned line re-throws every poll and
  stalls everything after it.
  8. HIGH — the "partial read" fix trades re-reading for silent data loss
  (log-tailer.ts:160, spool/tailer.ts:168). The cursor now advances past an
  unterminated line whose only copy is in memory → daemon restart loses it. The
  spool variant duplicates the partial prefix ({"id":"abc{"id":"abc123"}),
  failing JSON.parse and dropping real envelopes. A split multi-byte UTF-8 char
  also drifts the byte offset and skips the next line.
  9. HIGH — token usage double-counts on replay/crash (DatabaseV5.ts:165 engine,
  :623). The cursor is re-read from the DB each poll but only written after the
  whole batch; re-parses generate fresh envelope UUIDs so dedup never fires.
  The token insert and its dedup marker aren't in one transaction and
  ai_tool_events has no unique index — a crash between them double-counts
  permanently.
  10. HIGH — dedup is an unindexed json_extract full-table scan per event
  (DatabaseV5.ts:711). O(n²) overall; a busy tailer slows to a crawl.

  Features wired but dead end-to-end

  11. HIGH — extension→daemon session forwarding stores nothing
  (src/client/DaemonClient.ts:102). The daemon's ingest only handles ai.* types;
  session.updated/ended hit default: return false — yet every envelope is still
  appended to the spool and re-broadcast. ~43k never-ingested lines/day (~20
  MB) that get replayed and re-discarded on every restart. session.ended is
  never even emitted (dead branch).
  12. HIGH — hook ingestion is 100% rejected (hooks/forward.sh). Hook events use
  evidence type hook_metadata, which isn't in the protocol enum → validator
  drops every one. The "hook-first architecture" ingests nothing.
  13. HIGH — desktop can't authenticate in dev or on Windows
  (desktop/src/lib/daemon.ts:27, tauri.conf.json:13). Dev origin
  http://localhost:1420 is refused by bootstrap (and VITE_DAEMON_TOKEN is
  undocumented); Windows prod origin http://tauri.localhost is rejected by both
  CORS and bootstrap — shipped Windows builds can't reach the daemon at all.
  14. HIGH — OneDrive sync broken by the hardening diff
  (src/storage/sync/providers.ts:339). maxRedirects: 0 rejects Graph's 302
  download redirect — pull fails for every existing OneDrive user.
  15. HIGH — goal milestone notifications fire a startup burst then never again
  (src/tracker/TimeTracker.ts:603). On activation previous=0 replays all passed
  milestones at once; the reached-set is never reset on day/week rollover, so
  yesterday's milestones block today's.

  Testing & CI (the green checkmark is fake)

  16. HIGH — the test stage can never fail: xvfb-run npm test || true
  (Dockerfile:41). Combined with a missing .dockerignore (host sqlite binaries
  clobber the Linux build), extension tests almost certainly fail in Docker and
  nobody can tell.
  17. HIGH — no GitHub Actions workflow exists; the platform workspace and its
  18 tests are outside every CI entry point. Regressions merge to main
  unchecked.
  18. HIGH — zero tests on the riskiest code: log-tailer cursor math, DatabaseV5
  ingestion (914 lines), HTTP auth, snapshot restore + path-guard, the glob
  engine's real patterns. The Claude fixture is shaped to the parser, not to
  real logs — so model/cost attribution would silently return undefined on real
  ~/.claude transcripts while tests stay green.

  3. What it'll take to make it perfect

  P0 — correctness & security, before anyone runs this for real

  - Reject non-loopback Host headers on every route; make missing-Origin
  bootstrap return false; hand the desktop shell the token via a 0600 file read,
  not over HTTP (kills findings 1 + the local-curl path).
  - Constrain snapshot projectRoot to a server-side registry of real workspace
  roots; re-validate at restore time (finding 3).
  - Fix the glob baseDir math + sort-before-cap + per-file try/catch + bound
  timestamps and wrap per-line parse (findings 4–8). These are the
  data-integrity core.
  - Make ingestion idempotent properly: deterministic envelope IDs from
  (scanner, file, lineHash), envelope-id column + UNIQUE index, cursor written
  in the batch transaction (findings 9, 10).
  - Fix the hook evidence type and persist coding sessions in
  ingestEnvelopeFromSpool, or stop forwarding (findings 11, 12).

  P1 — make it real

  - Implement ed25519 verification and reconcile the two contradictory
  validators so signed manifests can actually load (finding 2 + its inverse).
  - Add .github/workflows/ci.yml (extension lint/typecheck/compile, extension
  tests under xvfb, cd platform && npm test); remove || true; gate merges
  (findings 16, 17).
  - Add the high-value tests: tailFile cursor scenarios, DatabaseV5
  ingest/dedup, HTTP auth, path-guard/restore round-trip, glob real patterns —
  with captured real log fixtures (finding 18).
  - Fix Tauri dev/Windows auth (useHttpsScheme: true + dev-token path) and
  OneDrive redirects and the milestone notification windowing (findings 13–15).

  P2 — productionize: privacy tiers + HMAC path hashing (paths are stored
  cleartext today), registry update channel + CDN fetch, launchd/systemd service
  install ("runs on login" is unimplemented), WebSocket live updates in
  desktop/extension (everything is fetch-once today), and a documentation truth
  pass (README still describes only the extension).

  - DNS rebinding closed: every HTTP route and WS upgrade now rejects
  non-loopback Host headers; /v1/bootstrap no longer hands the token to
  missing-Origin callers (exact allowlist: tauri://localhost,
  https://tauri.localhost, http://tauri.localhost). Token comparison is now
  timing-safe.
  - Snapshot arbitrary-file-write closed: projectRoot must be an existing
  directory containing .git (or in CODEPULSE_ALLOWED_ROOTS), never
  home/filesystem root; the canonical root + relative path are stored in the
  blob and re-validated at restore before any write. Old blobs fail closed.
  - Signature chain is now real ed25519, not a stub:
  platform/scripts/sign-manifests.mjs generates a gitignored dev key, all 6
  catalog manifests are signed, the pinned public key verifies them; unsigned
  official/verified manifests hard-fail, unsigned community is never
  auto-enabled. The protocol/runtime validator contradiction is gone.

  Ingestion (Fable 5) — glob baseDir off-by-one fixed (patterns anchor
  correctly, ** can't escape), newest-by-mtime selection before the file cap,
  per-file error isolation so one bad file can't stall the loop, timestamp-crash
  guards, raw-byte partial-line handling (no more UTF-8 cursor drift, no data
  loss on restart, no spool prefix duplication), deterministic envelope IDs +
  envelope_id UNIQUE columns with transactional batch+cursor writes (replays now
  dedup instead of double-counting), and session.updated/ended are actually
  persisted — the extension forwarding dead-end is closed from both sides.

  Extension — OneDrive 302 handling restored (auth header still stripped on
  redirect), milestone notifications keyed by day/week window (no startup burst,
  no permanent suppression), on-change session forwarding with reconnect
  probing, final session.ended emitted on end and on back-to-back rotation,
  forward.sh hook envelopes now pass validation.

  Tests & CI — platform suite grew 20 → 58 passing (tailer cursors, glob
  regressions, DB dedup, HTTP auth incl. bootstrap/Host cases, path-guard +
  restore round-trips, hook envelope); extension suite 33 → 45 passing;
  .github/workflows/ci.yml created (3 jobs, gates on push/PR); Dockerfile || 
  true removed and platform stage wired in; .dockerignore added; ci-local.sh
  stages made real.

  Verification

  Platform build clean, platform tests 58/58, root typecheck clean, lint 0
  errors (115 pre-existing warnings), CI YAML/shell validated, and extension
  tests 45/45 re-run by me after the last code change. The Opus security
  re-review + Fable correctness review of the diff found 12 issues; the one HIGH
  (dead setSessionEndedHandler wiring) was fixed in the automatic fixup round.

  comparison as HTTP, with exactly one shared implementation in cors.ts
  (verified by grep: no second copy).

  Daemon ingestion (Fable 5)
  - Failed ingest transactions no longer drop log lines: the engine resets the
  tailer's in-memory state so the next poll replays from the persisted cursor,
  with deterministic IDs deduping the prefix.
  - Envelope IDs now include each line's byte offset (computed with raw-byte
  arithmetic, so multi-byte UTF-8 can't skew them) — byte-identical lines no
  longer undercount tokens, while replays of the same physical line still dedup.
  - All DatabaseV5 writes funnel through one serialization queue, using
  AsyncLocalStorage so statements inside an open transaction don't self-deadlock
  — a concurrent snapshot write can no longer silently die in someone else's
  rollback. There's a true regression test for this.
  - ProcessWatcher installs its interval before the first poll — a transient ps
  failure no longer kills process detection for the daemon's lifetime.
  - SpoolTailer reads in 256 KiB capped chunks, and a full chunk with no newline

  The round-2 reviewer found a HIGH flaw in our own recovery fix: a second batch
  already queued behind a failing one could still commit a cursor past the
  rolled-back lines. The fixup agent closed it with a generation counter —
  resetState bumps the file's generation, and the engine drops any
  stale-generation batch at dequeue time, so only the post-reset replay can
  advance the cursor. It re-verified everything after (and I re-ran both suites
  again just now).

  What's still on the books (minor, reported not fixed)

  The review's non-serious residue: unqueued DB reads can still observe
  uncommitted transaction state on the shared connection (MEDIUM), the weekly
  high-water mark never decays so a genuinely new week's milestones stay quiet
  until VS Code restarts or goals change (MEDIUM), envelope IDs don't include a
  rotation generation so a rotated file's identical line at the same offset
  dedups wrongly (LOW), and probeReconnect flips mode before flushing buffered
  events (LOW). Plus known followups: spool compaction (blocked on forward.sh
  writing the spool from outside the daemon process), and Google Drive/Dropbox
  not yet using the shared redirect helper.

  Each review pass is now finding strictly smaller fish — this is converging.

  the extension package, and bumped the version to 1.1.0.
  2. The daemon is still the stale build: the installed extension's DaemonClient
  will connect to whatever codepulse-d is running. The instance from yesterday
  predates all the fixes, so when convenient: kill $(lsof -ti :7842,:7843) && cd
  platform/apps/daemon && node dist/main.js (current dist/ is freshly built).
  Also remember old unsigned manifests in ~/.codepulse will be quarantined by
  the new ed25519 verifier — delete the cached manifests once so the daemon

⏺ Fixed. Analytics removed — categories are now ["Other", "Visualization"] (both
  valid). Repackaged (code-pulse-tracker-1.2.1.vsix, categories verified in the
  manifest), reinstalled locally, committed (a4342e1) and pushed.
